The final steps to finalizing your unclaimed property claim are providing your identity and ownership of the asset. We recommend that you send a government issued ID card (Driver's license, passport, etc...) along with your contract agreement, but with all the scam in our society today, we understand that your might not feel comfortable doing so.

The Notarized Sworn Statement form can be used as an alternative way to prove your identity. Please mail the original copy back to us.

If the claim involves a deceased owner, please fill out the Estate Affidavit and mail the form back along with the LPOA and all other relevant documents needed for your claim.
These forms verify your relationship with the owner and will validate your claim.
